Reflective Account on Pressure Ulcers
There are four questions that is coming in the reflective account template. Lets see one by one.
What was the nature of experience in your practice?
Patient developed a ungradable pressure ulcer during the course of treatment.
What did you learn from this experience (Pressure Ulcer) in your practice?
I came to know importance of regular change in patient’s position and role of maintaining good personal hygiene for the patient. We also learnt importance of reporting the pressure ulcers early so that we can prevent the worsening earlier. Importance of grading of pressure ulcers. We also came to know when and where we are supposed to use incontinence pads and when we should not use the incontinence pads. We also learnt about the timely use of dressings in treatment of pressure ulcers.
How did you change or improve your practice as a result?
Pressure ulcers (also known as pressure sores or bedsores) are injuries to the skin and underlying tissue. This injuries are primarily caused by prolonged pressure on the skin. They can happen to anyone. But usually affect people confined to bed or who sit in a chair or wheelchair for long periods of time. After the incident, I made sure all of my patients who are prone to pressure ulcers are regularly turned every two hours. If the patient has got a grade 2 pressure ulcer on the sacrum and is incontinent, its best to insert a urinary catheter. This helps to avoid moisture in the area which can make the pressure ulcer worse.
I am now doing the incident report of all the pressure ulcer once they are grade 2. I am also referring the patient to the tissue viability nurse in accordance with the trust policy. When the patient is in grade 2 pressure ulcer stage, use of incontinence pads and pressure relieving mattress is according to the tvn advise. I learned that it is the duty of the nurse to report the incidence of pressure ulcers to next of kin as and when it develops.
